Care Coordinator

Job Description

Participate in staff recruitment, training and monitoring of all staff practices 

Development and maintenance of quality care to all service user.

Assess new and existing service users and their care plans, through incorporating client, care staff, and family views, thus promoting participation and independence.

Carryout 6 monthly and yearly reviews with existing and clients ensuring support plans meets service users changing needs.

Liaise with key worker for planning of schedules and submission of schedules to the scheduler on time.

Provide quality report on care provided to service user

Participate in investigations and disciplinary proceedings 

Liaise with the managers on planning new staff inductions and on the job trainings.

Liaise with manager in providing supervisions, competencies, and appraisals for care staffs timely

Ensure all necessary paper is correctly recorded and updated in service users files and on the App

Ensure alerts, concerns, late, and missed visits are flagged on the apps are investigated and a resolving action taken.

Provide information to managers on support to all staffs in terms of shortages, and be willing to join the care staff and offer helping hand in terms of crisis.

Liaise with other care agencies and multidisciplinary teams such as social services, GP, district nurses regarding service users care as required.
